Epidural Steroid Injection

An ESI or Epidural Steroid Injection can be described as a minimally invasive procedure done to alleviate pain in your arm, neck, and leg, and pain caused by the swollen spinal nerves due to a disc herniation or spinal stenosis. During this procedure, the required medicines are injected into your epidural region, a fat-rich area present between the protective sac of spinal nerves and the bone. Pain relief from ESI can last for weeks and even years. The main goal of epidural steroid injection is to reduce pain so you can quickly resume your daily activities easily.

If you are suffering from a herniated disc, these injections can reduce your symptomatic pain. The relief from pain can last from weeks to months and allow your back to get better with exercises and physical therapy. 

Several conditions can be treated by administering an epidural. Epidurals work well in conditions like spinal stenosis, radiculopathy, spondylolisthesis, degenerative disc disease, and sciatica. Certain types of epidurals (or selective nerve root blocks) can also be used to assist in determining the correct level for spinal decompression if surgery is required as a treatment for pain due to a herniated disc. Are all ESI the same?

There are three types of epidural steroids: Transforaminal Epidural Steroid Injection, Interlaminar Epidural Steroid Injection, and Caudal Epidural Steroid Injection.

  • Transforaminal Epidural Steroid Injection or Selective Nerve Root Injection
    A transforaminal epidural steroid injection uses therapeutic steroid and local anesthetic injected into the sheath surrounding a nerve root in the spine to decrease your pain temporarily and to define it more precisely. The procedure uses therapeutic steroid and local anesthetic (numbing medication) to decrease pain and inflammation. Pain relief from the procedure varies from minimal to long-term, depending on the specific symptoms.

  • Interlaminar Epidural Steroid Injection
    Interlaminar Epidural Steroid Injection refers to a procedure that involves the delivery of medications to the epidural space by directing a needle between the laminae of two adjacent vertebrae. The goal of this interventional pain medicine procedure is the relief of radicular pain and may be performed in the cervical, thoracic, or lumbar spine.

  • Caudal Epidural Steroid Injection
    Caudal Epidural Steroid Injections are steroid injections that are given at the base of the spine to treat chronic lower back and leg pain. They are extremely helpful in patients that continue to have back and leg pain after previous spine decompression surgery

Though ESI is not known to have any critical risks, certain long-term complications may occur as a result of continuous steroid admiration. These include bone demineralization, suppression of the HPA or hypothalamic-pituitary-adrenal axis, impact on the immune system, and higher glucose levels.

Several types of research have shown the efficiency of ESI injections for back and extremity pain. Additionally, the effectiveness of these injections improves significantly when therapies such as exercises and physical therapies are utilized in parallel to treat back and extremity pain
